Data Subject Application Form Pursuant to Law No. 6698 on the Protection of Personal Data Law No. 6698 on the Protection of Personal Data ("PDP Law") grants data subjects ("Applicant"), defined as data subjects in the PDP Law, the right to make certain requests regarding the processing of their personal data, as stipulated in Article 11 of the PDP Law.
In accordance with the first paragraph of Article 13 of the Personal Data Protection Law (KVK Law), applications regarding these rights must be submitted to Fleur D'orée Parfums, the data controller, in writing or through other methods determined by the Personal Data Protection Board ("Board"). In this context, applications submitted "in writing" can be made by printing out this form and submitting it personally, via a notary public, or by sending it to our registered email address signed by the applicant with a "secure electronic signature" as defined in the Electronic Signature Law No. 5070. In accordance with the second paragraph of Article 13 of the KVK Law, your applications will be answered within thirty days from the date of receipt, depending on the nature of the request. Our responses will be delivered to you in writing or electronically, as required by Article 13 of the KVK Law.
